DECLARATION OF AGE
Requirements for Age Declaration Document:
1. Valid Nigerian Passport of the applicant.
2. Valid Nigerian passport of the Declarant.
3. Declarant must be six years or older than the applicant.
4. Photocopy of the passport data page of the declarant and Applicant.
5. Two passport-size full-face photographs of both applicant and declarant.
6. $60 Age declaration fee.
7. Prepaid self-addressed envelope.
