Age Declaration

REQUIREMENTS FOR AGE DECLARATION DOCUMENT:

1. Valid Nigerian Passport of the applicant.
2. Valid Nigerian Passport of the Declarant.
3. Photocopy of the passport data page of the declarant and Applicant.
4. Two passport-size full-face photographs of both the applicant and declarant.
